Iowa community health worker training eligibility is restricted to organizations operating in counties designated as medically underserved by the Health Resources and Services Administration. Thirty-seven of Iowa's 99 counties meet this criterion, with population densities below 20 persons per square mile in 22 of those counties. Applicants must demonstrate existing referral relationships with at least one rural health clinic or critical access hospital.

Training curricula must incorporate Iowa-specific Medicaid managed care requirements and the state's community health worker certification standards administered through the Department of Public Health. Participants are required to complete 100 hours of didactic instruction plus 160 hours of supervised field placement in the applicant's service area. Proposals document the percentage of trainees who will be recruited from the same counties where services will be delivered.

Application scoring awards points for plans that address Iowa's aging demographic, where 18.5 percent of residents are over 65, and for documented partnerships with community colleges that award college credit for the training sequence. Unlike Nebraska applications, Iowa requires evidence that training sites maintain active enrollment agreements with at least two Medicaid managed care organizations operating in the proposed service region.

Quarterly reporting tracks trainee certification pass rates on the state exam and the number of client encounters logged during the supervised placement period, with a minimum threshold of 80 percent certification within six months of cohort completion.
