Children in the northwest and southeast quadrants, where farm employment predominates, encounter the highest per-visit expenses. Families rely on seasonal income from corn, soybean, and pork production, making even modest copays prohibitive. State claims data show that 24 percent of Medicaid-enrolled children in these counties receive no dental service in a given year.
Competition-based funding supports low-overhead group events that deliver screenings, fluoride, and education at aggregated scale across multiple small towns. Events rotate through county fairgrounds and consolidated school sites to achieve volume sufficient to lower per-child costs below $95.
Iowa requires explicit per-child cost modeling that accounts for winter travel distances exceeding 60 miles between sites and for equipment transport over gravel roads.
